It contributes to your physical and mental wellbeing and enhances your independence. Our services include transportation and dedicated disability support workers who help you make the most of the opportunities in your local community.
Supporting you out and about
We help to connect in person with your friends, family, colleagues and therapy services outside of your home. Based on your NDIS plan, we’re here to lend a hand with activities in the community that match your objectives. Here are some of the ways we can help:

How we Work together to support you
We are on your team. Here are our four steps of how we collaborate to support you to live your best life. You are the heart of everything we do.
Fill out the Client Intake Form
You, your carer or Support Coordinator fills out the Client Intake Form. We will get in touch to get the process started. We’ll let you know how our intake process works. So you know what to expect every step of the way. You can let us know if you have other supports that we need to work in with.
Develop a Support Plan
Together we explore your unique skills, strengths, and needs. We listen to you, you, your family or advocate, and your support professionals to develop. To plan the very best support for you, we might need to meet you face-to-face or do detailed assessments. We’ll agree on a support plan that suits you.
Our support workers assist you
Once you’ve confirmed the Support Plan that meets your NDIS goals, we’ll set up a Service Agreement. And then our support workers will provide the supports in your plan. We choose support workers who care. And we train them so they can skilfully support you. So you can be confident of receiving high standards of care, delivered with warmth, humour and heart.
Ongoing review and follow-up
We regularly review your Support Plan to ensure that we are meeting your needs and personal requirements. We welcome your feedback. Your Support Workers are the people who have the most insights. So we ask them too. We can also write reports if you need those. We often write reports for clients when their NDIS plan is coming up for review.
Find Out More About Our Community Participation Services
Here are some of the more common questions people have about our services. We know that everyone has individual needs so please contact us with any other questions.
How can I get help choosing the right community and support services for me?
Developing your support plan is a consultative process between you, us, your carer and other relevant people. Once your support plan has been put together and you are happy with it, we will ask you to sign off on it. We will provide you with a copy of your support plan.
How will I know that the services that I am receiving from Caring Lotus are helping me?
Our Support Workers or Clinical Care Manager will collect information when they work with you. We put all this information in your records. That helps us to check that our service delivery meets your current needs, interests and aspirations. Caring Lotus also does regular assessments with you to review your Support Plan. We make sure the support you are receiving still meets your needs.
What happens if I no longer want Caring Lotus services or if I need to take a break from my support services from Caring Lotus?
It’s always your choice. When using our services, you may stop them for any reason, and at any time. You just need to provide us with the length of notice included in your Service Agreement. If you’re leaving Australia to go on holiday or for another reason, you can give us notice to temporarily suspend your services with us.
